School Entry Requirements

Check boxes 1 to 7 to see which immunizations your child requires to enter school. (Boxes A and B are not required, but are highly recommended).

1. All Students
All students, grade Kindergarten through twelfth grade at the determination of risk by their physicain determine if they require a TB test on admission.
TB tests are to be given by physician with a child's 1st grade CHDP physical if they are determined at risk for TB by the physician.

2. Students New to California
For first time California registrants:
Varicella (Chicken Pox),
1 shot if 12 years or under when first dose given
2 shots if 13 years or above when first dose given

3. Kinder thru 9th
1 varicella vaccine (2 if given at 13 years or older)
The Department of Health Services recommends 2 varicella vaccines for all ages

A. Recommended for 2nd
Hepatitis A
Flu
Pneumonia
2 Varicella

4. Kinder & 1st
5 DPT shots (at least 1 on or after the 4th birthday)
4 Polio shots (at least 1 on or after the 4th birthday)
2 MMR vaccines (both on or after the first birthday)
3 Hepatitis B vaccines
1 varicella vaccine (2 recommended)
A TB test if recommneded by physicain

5. 2nd-12th (age 7 & above)
3 polio vaccine (at least 1 on or after the 2nd birthday)
3 Td, DT, or DPT combination (at least 1 on or after the 2nd birthday)
2 MMR (both on or after the 1st birthday)
3 Hepatitis B vaccines
1 varicella if 12 or under (2 varicella if age 13 or older.

6. 7th-12th
1 Tdap booster given at age 7 or older

7. Seniors
If you are planning to attend college, all above vaccines will be required. Start your Hepatitis A series now as they take 6 months to complete. If you plan to live in a dorm, the meningitis and influenza vaccines will also be required
